Registration Information

MAUE New Student Registration Packet

 
The documents listed below need to be submitted to complete the registration process and returned to your student's school to fulfill the enrollment requirements. 
Required Documents for New Student Enrollment:
  1. Current report card from pervious school
  2. Withdrawal form from previous school
  3. Completed enrollment packet Click here
  4. Mississippi Immunization Form 121 (original from your doctor’s office)
  5. Birth Certificate
  6. Social Security Card (optional)
  7. Custody Papers (if parents are divorced or parent not listed on the birth certificate)
  8. Behavior summary from previous school
  9. Must provide warranty deed and lease, if applicable (all occupants must be listed on the lease agreement and the agreement must be current)
  10. One bill in the parent’s name showing the street address and dated within the last 60 days:
    • Water
    • Gas
    • Electric
    • Cable/internet
    • Cell phone bills are NOT accepted.
